Open lower limb fractures in the UK trauma system: A multicentre prospective audit of current practice

Introduction Open fractures represent limb-threatening and life-changing injuries. Clear standards define how patients with these injuries should be managed in the UK. The study of open is, therefore, a key measurable example major trauma management as whole. This was conducted to characterise demographic, assessment treatment sustaining lower limb across UK Regional Trauma Networks. Methods A prospective, multicentre, audit according prespecified protocol against relevant British Orthopaedic Association (BOA) & Plastic, Reconstructive Aesthetic Surgeons (BAPRAS) for Trauma. All hospitals treating adults were eligible take part study. Patients included injured during six month collection period at each site. Results 3 Major Centres (MTCs) 5 Units (TUs) enrolled, data collected by 24 collaborators. 239 included, 11 had bilateral fractures. There 38 patient datasets from TUs 201 MTCs. predominantly male high energy 31.3% MTCs transferred another facility. Antibiotics given 41.7% within 1 hour. 74.4% limbs splint applied emergency department. 85.8% documented orthoplastics plan. injury their wound debrided 12 hours. Discussion 42.3% our cohort some point TU, indicating triage required network. Due sampling, we may under-estimating number passing through TUs, however, have demonstrated that this exists. These are under-represented many other studies registries such Audit Research Network (TARN) due funding model privileges Conclusion gives extensive new insight into demographics UK, demonstrating widespread involvement consistent deviation national standards.
